Health Activity Reports – Due Now

It’s tax time which also means it’s Church reports time (sorry ’bout that!).

There are three reports we would love for you to complete. 

1. Health activites annual report – this is a report of all health activites and programs your church has conducted between July 2010 and June 2011. You can dowload the paper forms below or complete the form online.

2. Health program highlights and interest stories – if you have any great stories or highlights from your programs we would love to hear about them. Again, you can download the form below or complete the form online.

3. Upcoming events form – we would love to know what Health events your Church is planning. We can then promote and/or assist your event. Download the form below or complete it online.
